Services

CSAH manages the Homeless Management Information System (HMIS), which tracks client-level information and services provided to people experiencing homelessness in Chatham County and the City of Savannah. This system helps ensure accurate data, supports better planning, and guides the effective use of resources in our community.

We lead the Coordinated Entry system, ensuring households experiencing homelessness have fair and consistent access to housing resources. Through a personalized assessment process, we work with each household to understand their needs and connect them to the housing and support services that best fit their situation.

As the Collaborative Applicant for the HUD Continuum of Care (CoC), we administer the annual CoC Competition Notice of Funding Opportunity (NOFO), supporting local agencies in securing federal funding for housing and homelessness programs. We lead the annual Point-in-Time Count, a communitywide effort to gather a snapshot of homelessness in Chatham County. This count provides vital data that guides planning, funding decisions, and strategies to reduce homelessness.

Notice Of Funding (NOFO)

The HUD Continuum of Care (CoC) Program Competition occurs annually. The Chatham/ Savannah process begins with Registration of CoCs by Chatham Savannah Authority for the Homeless, followed by the CoC’s review of Grant Inventory Worksheets (GIW) and the release of the CoC Program NOFO. The FY Competition ends when the final funding announcement is released by HUD.

Chatham Savannah Authority for the Homeless, as the Collaborative Applicant for the Chatham/Savannah ICH/CoC, is responsible for overseeing this process, which includes a local competitive application process and the submission of the CoC Consolidated Application to HUD, which consists of the CoC Application, the CoC Priority Listing, and the Project Applications.

This process involves the Continuum of Care (CoC) community and is not just about writing a grant.  It gives Chatham Savannah Authority for the Homeless an opportunity to showcase the collective impact of all partner agencies in the CoC while highlighting the strategic initiatives and approaches implemented to help us reach functional zero.  While the HUD NOFO competition can be complex and involved, it is an important process we hope you consider taking part in.

Please contact Jenn Dulong at jdulong@homelessauthority.org or Kay Zukowski at kay@homelessauthority.org with any questions regarding the HUD CoC Program NOFO.

Point-In-Time Count (P.I.T.)

What is the PIT?

  • Chatham Savannah Authority for the Homeless leads the Point-In-Time Count for the ICH/CoC. The Point-in-Time (PIT) Count is a real-time survey of sheltered and unsheltered people experiencing homelessness on a single night, providing a snapshot of the number of people experiencing homelessness in our community.
  • The PIT Count helps inform federal, state, and local decision-making and resource allocation to effectively address the experience of homelessness. To receive federal funds, the U.S. Department of Housing and Urban Development (HUD) requires that communities perform a PIT count annually of people who are experiencing homelessness. This count is done annually for those who are sheltered in emergency shelters, transitional housing, and Safe Havens on a single night.

What the PIT is NOT?

  •  The PIT Count excludes people staying with others temporarily (couch-surfing) or instances where multiple households share a single living space. Counts also exclude people in institutional settings (e.g. Hospitals, jails), even if they experienced homelessness the prior night.
  • The PIT does not reflect the full, annual extent of homelessness in Chatham County.

Coordinated Entry (CE)

Chatham Savannah Authority for the Homeless is the director and manager of Coordinated Entry for the Chatham/Savannah ICH/CoC. Coordinated Entry is a streamlined system that assesses individuals and families experiencing homelessness using an objective tool that identifies vulnerability, services needed and barriers to housing. Those who are assessed as having the highest vulnerability and housing barriers will be prioritized for access to available emergency shelter beds and housing programs as vacancies occur. The coordinated entry process begins at the first point of engagement and supports unhoused community members on their journey “from streets to stability”.

Through homeless service access points, individuals can be assessed and prioritized based on vulnerability and severity of needs. Currently, the CoC prioritizes permanent supportive housing for chronically homeless clients assessed through coordinated entry. Coordinated entry is essential for clients who need rapid rehousing intervention.

Community Information Exchange: Innovative Community Work Improving Outcomes

Our community is the first in the US to receive approval for integrating HMIS into our Community Information Exchange Model. This work, initiated by our Chatham County government partners, allows key community partners from healthcare, behavioral health, first responders, emergency services, and homeless services to access and share client level data.

The Chatham/Savannah Continuum of Care uses a Homeless Management Information System (HMIS) to collect client-level data and data on the provision of housing and services to people experiencing homelessness in Chatham County and the City of Savannah. The HMIS system mandated by Georgia Department of Community Affairs is CaseWorthy. Agencies receiving funding from the Chatham/Savannah CoC are required to use CaseWorthy to manage the progress of clients receiving CoC services and interventions. Chatham Savannah Authority for the Homeless is the director, manager, and lead agency for HMIS for the Chatham/Savannah CoC. Access to the HMIS data platform is made available to general members of the ICH/CoC at no cost. For technical assistance and inquiries to become an HMIS user, please see the contact information below.
